Legal Requests

Legal requests for billing records require the following:

-A signed release from the patient
-A fee of $20 made payable to Lake County Radiology Associates, S.C.
-Patient information including name, DOB, address and the specific date of service for the bill that is being requested

-Questions regarding requests can be directed to:1-586-412-4340 Mon-Fri 8:30am-5pm EST.

-If all of the information including the fee is received, the normal turnaround time for legal requests are 5-7 business days.

-Please note: If we do not receive an authorization and the check we cannot process the request. At that point we will send the request or fax the request back to the lawyer to obtain the missing piece.
